Program Enrollment and Interest Forms

When you submit an enrollment interest form, registration form, or contact form on our website, we collect information you voluntarily provide. This may include your name, email address, phone number, the name and age of the participant, disability or developmental diagnosis (where relevant to program placement), and any notes or questions you include. This information is used solely to respond to your inquiry, enroll participants in appropriate programs, and communicate program updates.

Our forms are hosted and processed by JotForm, Inc. (jotform.com). When you submit a form, your data is transmitted to and stored on JotForm's servers in accordance with their privacy policy, available at https://www.jotform.com/privacy/. JotForm is HIPAA-eligible and compliant with applicable data protection regulations. We do not use your form submissions for marketing purposes or share them with unaffiliated third parties.

Data Involving Minors and Individuals with Disabilities

Because UDID Academy serves children and individuals with developmental disabilities, some of the information we collect may include sensitive details such as a participant's age, disability diagnosis, Regional Center vendor authorization, or behavioral or developmental notes. This information is collected only when necessary to provide appropriate programming and is never sold, shared with advertisers, or disclosed to third parties except as required by law or as needed to deliver our services (for example, sharing relevant information with a Regional Center service coordinator at the family's request).

We do not knowingly collect personal information from children under 13 without verifiable parental consent. If you believe we have inadvertently collected such information, please contact us at [email protected] and we will promptly delete it.

Website Analytics — Google Analytics and Google Tag Manager

Our website uses Google Analytics 4 (GA4) and Google Tag Manager (GTM), services provided by Google LLC. These tools collect anonymized information about how visitors interact with our site, including pages visited, time spent on pages, general geographic region (country/state level), device type, and referring website. This data helps us understand which content is most useful to families and improve our website.

Google Analytics uses cookies and similar tracking technologies to collect this information. The data is processed by Google in accordance with their privacy policy at https://policies.google.com/privacy. We have enabled IP anonymization so that no full IP addresses are stored. Google Analytics tracking is only activated after you provide cookie consent via the banner displayed on your first visit to our site.

In addition to Google Analytics, our website uses a first-party analytics cookie (c2_analytics_consent) to track aggregate site usage such as page views and session duration.   • Enrollment and program records — retained for the duration of a participant's active enrollment and for up to three years thereafter for administrative and legal purposes.
  • Contact and inquiry submissions — retained for up to two years from the date of submission, or until you request deletion.
  • Analytics data — Google Analytics data is retained for 14 months per our GA4 configuration. First-party analytics cookies expire after 365 days.
  • Email and phone communications — retained for up to two years unless you request earlier deletion.

  • Right to access — You may request a copy of the personal data we hold about you or your child.

JotForm (Form Processing)

Form submissions are transmitted to and stored by JotForm, Inc. JotForm processes data on servers located in the United States and is subject to their privacy policy at jotform.com/privacy.

Google LLC (Analytics)

Anonymized analytics data is processed by Google Analytics and Google Tag Manager. Google may process this data on servers located outside your country of residence. See policies.google.com/privacy.

Regional Centers and Service Coordinators

With your explicit consent, we may share relevant participant information with your assigned Regional Center service coordinator to facilitate IRC vendor authorization or service coordination.

Legal Requirements

We may disclose personal information if required to do so by law, court order, or governmental authority, or if we believe in good faith that such disclosure is necessary to protect the safety of any person or to prevent fraud or illegal activity.

6. Phone Number Privacy Policy

Your phone number will be used exclusively to send you messages you have opted to receive based on your consent. We do not sell or share your personal information with third parties.

We respect your privacy and are committed to protecting your personal information.

Use of Information

We use your phone number to send SMS messages regarding your inquiry and customer care related topics. Mobile phone information or opt-in data collected will not be shared, sold, or disclosed to third parties or affiliates for marketing purposes.
